‘Mr. M’ Case Study

Profile: B. M. 46yo male, T2DM, extreme bariatric (754 lbs), immunocompromised, private insurance carrier.

Situation: In September 2021, Mr. M was answering the door at his apartment when he fell and pinned himself against his front door fracturing his right leg and injuring his lower back. The patient was admitted to a hospital, where he began his recovery. Within a few days however, he was ready for discharge to a SNF.

Problem: Unfortunately, the hospital was unable to find any facilities accepting SNFs in the state for months.

ATP HealthCare Solution: After several months of searching, the hospital contacted ATP HealthCare to search for short term rehab for Mr. M. within 72 hours ATP HealthCare had generated several out of state options for rehab. ATP HealthCare assisted the accepting facility, the hospital, insurance, and the transportation company. By creating several contracts that resulted in Mr. M being transported to the specialty SNF for rehab, ATP HealthCare was able to get the patient the treatment he needed. Two months later Mr. M was in his apartment and back to everyday life.

‘Mr. C’ Case Study

Profile: C, 55yo male, T2DM, Post-Stroke, Left Sided Weakness, Wheelchair Dependent, Opioid Dependent, Anti-Coagulant, Self-Pay.

Situation: In 2018, Mr. C suffered a stroke.

Problem: Mr. C had been regularly visiting his local hospital with multiple 2-3 month stays over the following 18 months after his original admission for a stroke.

ATP HealthCare Solution: C. Our team worked with the patient’s family and the hospital to assist in getting rehab at a Rehab Hospital under contract while Medicaid was being processed. We were also able to secure a ALF placement for him after his rehab. The ALF was selected because its DON was a peer. Mr. C had been divorced for 10 years and really needed that peer support to help him complete his social security and Medicaid waiver application, which we were also able to assist him in submitting.

Patient Becomes A Client: After a year at the ALF with social security and Medicaid waiver, Mr. C felt his care was too much for the ALF and requested our assistance in finding him a bed in a nursing home. We were able to quickly find him an appropriate long term care facility where he remains today, and his hospital visits have all but disappeared.

‘Ms. B’ Case Study

Profile: B. 26yo female, T1DM, community Medicaid

Situation: While walking her dog in June 2020, Ms. B had a seizure and was found unresponsive by a good Samaritan who gave her CPR. EMS arrived to take her to an area Trauma Center. EMS lost the patient twice enroute, they got her back prior to arriving at the Trauma Center, and she spent the next 6 weeks in a COVID Unit ICU bed. Ms. B had suffered a major brain injury and was given inpatient rehab.

Problem: Skilled nursing was the discharge recommendation, however when the hospital began searching for placement for Ms. B in August by October the hospital had not found a proper placement option. Even though Ms. B was wheelchair bound, unable to transfer, unable to make her needs clearly known, and unable to manage her medications she was then discharged to an unlicensed home with medication orders. Ms. B then suffered a Grand Mal Seizure about 4 hours after arriving at the non-licensed facility. Ms. B was then transferred to St. Lucie Regional Medical Center where she was admitted. Ms. B was able to receive continued inpatient rehab at St. Lucie Regional Medical Center until she was ready for discharge in late November.

ATP HealthCare Solution: No accepting facility could be found until ATP HealthCare was contacted in mid-March 2021. Within two weeks our staff had an accepting Transitional Rehab facility and accepting ECC-ALF. We also worked with the family to get Ms. C set up with social security and the proper insurances. Ms. C did complete her Transitional Rehab program; however, her kidneys had deteriorated rapidly, and she was then transferred into a SNF.
